The Elmhurst Bush Nursing Centre has developed the 2024-2027 strategic plan using statistical data from our community, survey results from our consumers and with the consultation from key stakeholders.

A change to our values and key priority areas have been identified based on communities needs as well as Governance Key Performance Indicators' and Board of Management continual improvement requirements.

This document will enable EBNC to continually review the current strategic plan and track Key Performance Indicators

37 Surveys were completed from the 250 surveys sent out to members of the Centre

100% of respondents felt that:

• they feel like they participate in their own care
• they feel included in their healthcare decisions
• they feel they are provided with information related to their care
• they feel valued and respected while in the Centre.

The annual survey also provided the Centre with feedback for improvements including:

• The need for more information sessions
• More inclusive activities for different cohorts
• An increase to doctor visits
• More community events
• Health promotion activities.

OUR PARTNERS

A board with blank paper was placed in the reception area for members of the Centre and

community to provide their feedback. This board was also relocated during events to better

capture the most feedback and responses as possible. The community was able to highlight their values and request for improvements of the centre.

Chronic Health and Delivery of Care

- Ensure the right qualifications, knowledge, and skills are available to provide safe, high quality health care to patients

- Communicate with other specialist areas about health care conditions so that individual health care needs are met

- Targeted health promotion and prevention to reach vulnerable groups in communities

- Empower consumers to make their own informed decisions to prevent and manage their individual healthcare needs.

Client Engagement and Health Literacy

- Feedback will be heard and actioned appropriately

- Consumers are safe and comfortable accessing health care

- Support and communicate choices to patients, carers, families, and consumers

- Give information in a way that can be understood and support clients in their decisions concerning health care
